“Honestly, I am continually grateful for your help in transforming our practice, and I can’t count how many people I’ve informally told how helpful your services are. I couldn’t be happier with how everything has worked out with implementing the changes you recommended. The practice is very financially strong, and we’ve grown to add a PA and other additional staff to offer esketamine treatments for Treatment Resistant Depression. I love getting to go to work every day, and am probably one of the least burned-out psychiatrist you’ll ever meet! And my worry that I wouldn’t be able to serve the sicker patients SMI actually turned out to be the opposite! Without being beholden to network contracts, I can offer sliding scales as I see appropriate, and am able to meet people with SMI wherever they’re at financially. I’ve actually found that my average acuity level actually went up! I realized that I had a fair number of people who had been doing great for a long time and more or less just needed refills of meds that anyone could prescribe, but they wanted to schedule relatively frequent followups, more or less just to “chat” with somebody, and with just a small in-network copay, they didn’t have the same motivation to detach from me that I had for them. But going out of network has made patients more thoughtful of whether they really need to see me, or if they can go back to their PCP or in-network psychiatrist. This keeps my caseload open to keep a relatively constant stream of more acutely sick patients— which is exactly what I want it to look like. On the flip side, I also have more psychotherapy patients, because it’s financially viable for me to do so. I really feel like I am living the dream!”
